The Premier Case Review

Healing takes time—and so does understanding the whole you. Our Premier Case Review is structured to uncover the deeper patterns behind your symptoms and guide you toward relief.

Here’s what you can expect when we work together:


Step 1: Complementary strategy session 

In this phone or video conference conversation, you’ll learn more about how our process works and if it’s a good fit for your specific situation.

Step 2: Deep dive intake form

As we move forward, you’ll complete a comprehensive intake form designed to surface patterns in your history, habits, and health. This is your opportunity to be fully seen—beyond your diagnosis.

Step 3: Premier Case Review - Part 1

Leveraging your intake form, we map your full health story—past and present—using the Functional Timeline and the ATMs (Antecedents, Triggers, and Mediators) framework. This is where we begin uncovering what’s really driving your symptoms and overall dysfunction. You may be asked to track certain lifestyle behaviors prior to our next meeting. 

(1 hour session, in-person or digital)

Step 4: Premier Case Review - Part 2

In this session, we begin translating insight into action using the Functional Matrix. You’ll walk away with an understanding of contributing factors driving your symptoms and how we’ll approach healing through personalized nutrition and lifestyle strategies. We meet you where you're at, evolving your care from there. 

(1 hour session, in-person or digital)

Step 5: Premier Case Review - Follow-up

Functional Nutrition is a dynamic process—our work together continues as we implement your protocol, track progress, and make adjustments based on your body’s responses. We revisit the Matrix regularly to keep you moving toward balance. 

(30 minute session, in-person or digital)

The first follow-up is included in the Premier Case Review. Additional sessions are available for purchase.

To get started on the healing journey, we ask you to request a complimentary strategy session and tell us about your health concerns. Once you fill out the form, we will be in touch within a few days to make sure that the care we provide fits your health needs.

For those clients local to the Pleasant Grove, Utah area, sessions can be arranged in person. For all others, we're happy to accommodate our sessions through digital means (phone or video conference).

To get started, please complete the medical questionnaire and consent forms specific to your appointment type. During the Premier Case Review, we may also ask you to track certain aspects of your lifestyle or share any recent lab work you have on hand. These details help us provide the most personalized and informed recommendations possible.

Our services are not covered by insurance, and we do not provide billing codes or NPI numbers, as this is outside the scope of our nutrition and lifestyle practice. Like many functional medicine providers, we operate as an out-of-pocket service in order to offer comprehensive, personalized care that isn’t limited by insurance restrictions. That said, some clients successfully submit invoices to their insurance for partial reimbursement or use funds from their HSA/FSA accounts. We’re happy to collaborate with your other healthcare providers and encourage you to explore these options if they’re available to you. Your wellness is our priority, and we’re committed to helping you get the care you need.

No, prescribing medications or diagnosing conditions is outside the scope of our practice as nutrition and lifestyle professionals. However, we’re happy to collaborate with your medical team and share insights from our perspective when it’s helpful to your care.
